I Michael H McMillan DMD, the legal owner, custodian and dentist of record, for all patient records located at Michael H McMillan DMD, 
PA does hereby announce the transfer of my practice and all associated patient records to Dr. Nicole Rasmussen as of June 1, 2026. Dr. 
Rasmussen and Rasmussen Family dentistry are the sole custodian and dentist of records located at 4215 Del Prado blvd. Cape Coral, Fl. 
33904. Office number 239-549-1117.
Any patient, or patient’s representative, by written request may obtain a copy of their dental records at nominal cost. We will 
adhere to the strictest standards of patient confidentiality and only patients of record, custodians of duly appointed legal 
representatives may obtain patient record copies. All original records shall be maintained for a period of 4 years from the date of the 
last patient examination or treatment rendered.
